Well, some do. Depends on the situation which mic is best.
There are a great many tricks and products available now
to attach mics to costumes (or under costumes). Hang around
over on news:rec.arts.movies.production.sound for a while
to read about many ingenious real-world applications. They
have things like "vampire clips" and "moleskins" and other
tricks depending on what kind of material, etc.

or mics hidden at the hairline, etc. etc.
