The PIE patch just modifies a Windows system file to redirect your WinMX
client to the new cache server locations (since the official cache servers
are now offline).

The DLL fix does a similar thing, in redirecting WinMX towards the new cache
servers, but does this redirection in the DLL itself rather than through the
Windows hosts file. In addtion to that the DLL also blocks media companies
from connecting to you.
