If you have a router or modem with NAT/firewall you'll probably need to
configure port-forwarding through that as well. Also check to see if
Windows XP, if you use that, has its in-built firewall enabled for your LAN
connection.

Both most likely to do with your TCP not being open to the outside world as
above. Sometimes it can be ISP related though...
