I did that and it is quite tedious. The grid snap helped. 
 
MF4 scores a big plus on this one, because I do not need a dummy 
thumbnail for the firstplay clip. But TDA gets honorable mention for 
its ability to edit menus.

No matter how you do it, you need a 13 title menu for 12 actual titles 
and a playfirst title. That means you have to edit the 13 title menu 
to look like 12 titles. Once it's done I suppose you could add it to 
the library somehow (like the Favorite in NF4).
