We just replaced our aging QTV rig with a new LCD one from Prompter
People.

They use machined aluminum blocks as camera risers to fit both smaller
ENG (DSR-300 level) cams and allow the mounting of smaller DV cams like
the PD-150, etc.
Look on their site under the 15/17 professional prompters and you'll
find the blocks listed as accessories.
They're just machined aluminum blocks that have been powder-coated black
to match the PP hardware, but they do the job and at about $40 they are
probably cheaper than the ticket cost of taking your own block to the
local power coating guys.
Then again, if you just want the function, cut yourself a piece of 2X4
and pull out a drill.
