They rarely are. What is usually shown on some films, where they clean
the audio with filters and computers and the voice comes clear, is pure
fantasy.
You equalize to pick certain frequencies and attenuate them. But
whatever is close to the voice range will affect intelligibility and
the voice quality.
That's why you use wireless mics, that can pick a cleaner dialogue if
well placed. Hyper-directional mics and good acoustics are essential on
some situation, but that's why studios are still the way to go on most
Hollywood productions.
But a completely clean location audio is hard to get and is hard work.
