It can be done with active circuits (which require power).
There are many circuits out there to do this if you are
talking about DIY, or there are boxes available to just plug
in if that is what you are looking for.
Or it can be done passively with transformers which are
expensive if you want ones that perform well. For example...
www.jensentransformers.com which are generally considered
to be the gold-standard in matching/isolation transformers.
