from what I can see the insert is called a swing tray, so I entered

swing double dvd case

into google and the one of the first finds was this one, so the term
appears to be the right one (check third case down the page)

I have seen Amaray advertising the trays on their own but they're
quite expensive when coupled with the cost of their cases
