You posted your lengthy question 3 times in less than 5 minutes, so
I'm not sure which one to respond to, so but I picked this one.
The problem you're describing is usually caused by dodgy media that
doesn't get along well with your burner, either because the media
itself is a formulation that your burner doesn't work well with, or
because you're burning at too high a speed. I skimmed your post a few
times but apparently I'm overlooking the media brand you're using and
the speed at which you're trying to burn.
As for the +/- question, to me the right way to answer that is by
working backwards. Which format is supported in the player(s) that you
intend to use? Next, if that format isn't supported by your burner,
get a new burner. Finally, use media that others have tried and have
gotten good results, such as Taiyo Yuden or Verbatim Datalife Plus.
