After a bit of research I have found this
" I just found another Tappy bit on the disc. If you notice in
the
movie, Tappy says his system involves three steps in his "Month of
Fury": The first is no red meat. The second is no sugar. Sara passes
out just as he is about to reveal the third step. In the audio
commentary, Director Aronofsky seems on the verge of revealing the
missing step, but then says, "nah, you'll probably be able to find it
on
a web page". This got me to look up the DVD Easter Egg Page at
http://www.dvdeastereggs.com/ Turns out there is another 2.5 minute
chunk of Tappy's infomercial hidden. To open it, go to the chapter
index on the disc. Highlight (but do NOT click) the Chapter 21-24 bar
(note it says "#3 shh" at the bottom). Now hit the up directional
arrow
button on your remote twice, then hit the "enter" button. #3 will be
revealed (at which point, I have to drop out of the program).

This answers all three quesitons immedatedly.
I hope you didnt spend as much time decyphering this as I did. grr, the
answer was so simple and yet so very well hidden.
