I am beginning to think that your DVD burner is not fully compatible
with CD-DVD Speed. The scans require looking inside the drive for
statistical information, and if your burner does not allow that, then
Speed won't let you do that test.
If you want to chase this down just for your own edification, go to
CDFreaks and enter the appropriate hardware forum for your make of
drive. Then ask about compatibility with CD-DVD Speed.
Since you trust your Plextor tests, and they do indeed show
considerable problems on the outer bands, I think you have the answer
to your original question, namely, that bad discs or burns or both is
what is causing the bad read for that one model player.
There is another possibility, namely that you are burning +Rs with the
latest spec and it is incompatible with the older spec in the player.
