A number of ways to do it - this what I would do with the gear you have.
1 Covert the vob files off the DVD to mpeg2. You could use Womble, VideRedo
or ProjectX http://savefile.com/projects/656375
2. Demux the mpeg2 file into sound and vision using ProjectX, a free
program.
3. Import these 2 files along with your subtitle track into
4. DVDAuthorGUI also a free program.

5. Burn the resulting VIDEO_TS folder to disc.
No menu's but as many chapter points as you want.
Everyone has their favourite way and I'm sure others will describe what they
do.
