You seem to have it worked out on your own already; the simplest thing
is to save each chapter out as it's own stream and have viewers click
on a link to each as gthey see fit. There are other methods too, but to
me this seems practical and simple.
My biggger worry though is, for medical images, if it's something like
surgery, streaming the footage may degrade it tot he point of
uselessness. A lot of that would depend ont he codecs used, the bit
rate, etc.
