It does happen a lot -usually it's someone playing one game with sounds from
a totally different one (pacman death-noise is almost always used).
Possibly it's something to do with various copyrights, but mostly I would
say it's to more do with how a director or stylist or whoever wants a scene
to look/sound like.
eg. "Now I need you guys to fiddle with some controllers... no I don't want
these ones... I want ones that look more like... ah yes, here are the ones I
want."
-ie. the actions and props of the actors are a seperate choice to what
someone else has decided is being played on the tv screen.
