The ID3v1 and ID3v2 tags aren't necessarily synchronized, so you should
make sure that they are both using the same tag type. Changes to the
ID3v1 file are very easy to see, since the ID3v1 is stored as the last
128bytes of the file -- you could use a Hex file viewer like the
freeware xvi32 to directly view that content.
One other thought is that some programs will keep the information in an
internal database, and will only scan the file content the first time
it is located.
