Sales Tax goes to the State, so unfortunately you'll need to apply State by
State. Go to the Department of Revenue's web site for each State you wish to
recover Sales Tax and see if they have a pdf for download (I doubt it) and
least you'll be able to find the contact address for that State.
Remember that this week is Tax Week for the US so those departments are
going to be very busy for the next month or so
