The true cause of your sporadic pixelated DVD backups

What is the cause of sporadic pixelated DVD backups?

I don't see any errors whatsoever using DVD Shrink and Nero to rip and
burn DVD backups yet roughly one out of five or ten archival DVDs has
random pixelation somewhere in the playback results using TDK DVD-R
1-16x 4.7 GB DVD media and a Pioneer DVR-S706 single layer DVD R/RW
CD-R/RW writer.

Why the random sproadic pixelations?

More specifically:
How did the random blocky pixelations get past the Nero verification
stage?
Where do these random pixelations come from (they don't seem to be on
the original)?
Is there a software scanner which will find them (other than playing
the DVD to completion)?
Is random pixelations a hardware or software or meda problem anyway?
